A crane tipped off its tracks on the Port Mann bridge construction project east of Vancouver on Friday morning and dropped a segment of the bridge into the Fraser River.

Bridge project spokesman Max Logan said the gantry crane, which is designed to move on rails, is secure and nobody was hurt, but a 90-tonne section of the bridge deck the crane was moving fell into the Fraser River.

The segment struck a pile as it fell and is now submerged in the river. It is unclear whether the segment can be recovered.

The bridge segments are brought in by barge and lifted into place by the special 720-tonne horizontal gantry crane, which is also called a segment launcher.

Logan said one of the supports for the gantry appears to have been crushed and the gantry pitched forward.

It is unclear how long the mishap will delay the project, but the contractor Kiewit is responsible for any costs that result.

The Trans-Canada Highway remains open on the old Port Mann Bridge and there are no reports of any injuries.

When completed, the $800-million bridge will provide 10-lane crossing of the Fraser River for the Trans-Canada Highway east of Vancouver. It is scheduled to be completed in 2013.

Crane falls at Mich. construction site; 3 hurt

GRAND RAPIDS, Mich.—

A 250-foot-tall crane collapsed Friday morning onto a building at a construction site at the Kent County jail complex in Grand Rapids, injuring three people, authorities said.

The jail was placed on lockdown following the incident. Kent County Undersheriff Jon Hess described the injuries to a construction worker and two Kent County jail trustees as minor, The Grand Rapids Press (http://bit.ly/ziaMEe) reported.

Both the department construction company officials said all inmates and workers were accounted for.

Construction company Owen-Ames-Kimball Co. said Friday in a statement it was investigating the cause of the accident and had no further information. Hess said it appears that a cable snapped as the crane was carrying cement.

Images from the scene showed part of the crane leaning against a building. A garage area where sheriff's vehicles are kept was damaged.

The main jail that houses inmates apparently wasn't damaged, and Hess said authorities were checking the structural integrity of that building.

The crane that fell has been used for seven months on a $27 million renovation project, Hess said.

This is the second fatal case involving heavy construction equipment falling on a vehicle reported this year.

PETALING JAYA: A man and a woman were killed after a piling machine fell on the car they were travelling in at the Jalan Meru industrial area in Klang this afternoon.

Bernama identified the dead as Abd Rahim Abd Rahman, 51, and his wife, Nurhayati Rosli, 46, an Indonesian national.

The victims, who were in a dark blue Perodua Axia, were killed on the spot.

This is the second fatal case involving heavy construction equipment falling on a vehicle reported this year.

In August, administration executive Chin Khoon Sing, 24, was killed instantly when a crane hook fell on her Perodua Kelisa from 20 storeys up at Jalan Raja Chulan.

Chin, who worked in Puchong, was on her way to join her family for dinner when the incident occurred.

Bernama also reported that the developer of the construction project in Bandar Baru Bukit Raja had been issued with a stop work order with immediate effect following the incident at 12.35pm.

Department of Occupational Safety and Health Director-General
Mohtar Musri said the stop work order was issued so that investigations could be carried out.

“DOSH officers are at the scene. The next course of action will depend on the outcome of the probe.”

Crane Collapses, Crashes Down On Phra Khanong BTS Station

A crane at a construction site located beside Phra Khanong BTS Station on Sukhumvit Road came crashing down on Sunday morning causing minor damage.

Emergency services were called to the scene of the accident just before 11am and so far there have no reports of any injuries.

Rescue workers found the crane topped over blocking the footpath and the inside lane of the road. During its fall, the crane hit the BTS building and stairwell causing minor damage to the concrete wall and smashing siding and handrail of the stairs.

Power lines were also brought down and a telephone cabinet smashed.

An investigations into the cause of the accident has been launched by city officials with initial reports suggesting heavy rain caused the ground beneath the crane to collapse sending it crashing.

Saturday morning at the site on Cherry street in Saratov, a construction crane collapsed along with the crane operator. A woman was seriously injured, reports «Interfax».

What caused yesterday’s crane collapse in downtown Reykjavik?

Authorities investigating the collapse of a construction crane in central Reykjavik yesterday suspect that the crane was lifting far too heavy a load – and that somebody had tampered with the crane’s safety gear.

Miraculously, nobody was hurt when the crane came crashing down over Reykjavik’s famous Bæjarins bestu hot....

Officials from the Administration of Occupational Safety and Health in Iceland (AOSH) have already announced the findings of their initial investigation.

“Firstly, the crane is in no way structurally set up to carry a load as heavy as it was [at the time of the accident],” says AOSH Director Eyjólfur Sæmundsson.

“The crane also displays physical signs that the in-built safety system which is supposed to shut down the crane and prevent the lifting of excessive loads may have been tampered with.”

A mechanical failure is being blamed for the dramatic collapse of a crane that left three workers dangling from a North Sydney building.

At least one worker suffered broken bones in the hair-raising incident and all three were recovering in the Royal North Shore Hospital on Sunday afternoon.

An element of danger still remains at the North Sydney site as authorities grapple with how best to dismantle the dangling crane piece by piece. They were still planning the recovery as of 10pm on Sunday night.

CFMEU NSW secretary Brian Parker said there was a chance part of Arthur Street would be closed on Monday morning, as it would take hours just to set up before they could start working on the crane.

"It is a very, very dangerous process and it needs to be analysed very carefully," Mr Parker said from the North Sydney site.

"This could be equal to, if not worse than, the damage that has already been done."

A team from Titan Cranes, which is also involved in the construction project at Barangaroo, was at the Meriton building site on Arthur Street on Sunday to deconstruct a tower crane as the project neared completion.

Something "has gone amiss" about 11.30am with the riggers involved in the process, Mr Parker said.

The crane's jib bent, collapsed and hit the side of the building.

The riggers - aged 24, 28 and 36 - who were working within the crane's boom, were trapped and dangled precariously about 60 metres above the ground.

"They had their safety equipment on and lanyards so when the boom came down they were strapped on," NSW Fire and Rescue Superintendent Adam Dewberry said.

The 24-year-old worker freed himself and climbed on to the building's roof.
Operators of a smaller crane at the same site then managed to lower down a metal bucket to their trapped colleagues, who were lowered to the ground.

Mr Parker said it looked like mechanical failure rather than human error was at play.

However, an initial look at the collapsed crane showed nothing obvious to indicate what exactly that failure was.

"The unfortunate thing here is we don't know exactly what has gone wrong," Mr Parker said..."

 A giant construction crane collapsed on a four-storey underground car-park in Tel Aviv on Monday morning, injuring dozens of people, some seriously. Another person was critically injured. "Rescue forces have extracted an additional body from the building that collapsed yesterday in Tel Aviv", an army statement said. Two people trapped in the structure had managed to call emergency services from the rubble. The Magen David Adom ambulance service said it was treating 10 people - including one with severe injuries. A construction crane fell on a car on the Tappan Zee Bridge Tuesday, causing at least one injury and closing the Hudson River bridge in both directions.

The crane landed on a car on the bridge around noon, according to some news reports; others have said it did not land on any vehicles but did cause injuries. State police announced that all seven lanes of the bridge were closed in both directions.

The bridge, which connects New York’s Westchester and Rockland counties, is a constantly crowded passage over the Hudson River. A new replacement bridge has been under construction beside the old bridge, several cranes have been helping to construct that new bridge.

Interstate 287, which includes the Tappan Zee Bridge, connects with Interstate 95 just south of Greenwich and the Hutchinson River Parkway just West of Greenwich, is a popular northern crossing of the Hudson River for Connecticut residents. It also links to Interstate 684, Interstate 87 and the Sprain River Parkway on the West side of the Hudson River, which are all popular north-south running roadways for residents of western Connecticut.

Crane collapses in Jalan Pahang hitting bus and two cars

PETALING JAYA: Motorists and pedestrians near Jalan Pahang, Kuala Lumpur, had a narrow escape on Tuesday evening when a crane collapsed and hit a bus and two cars.

City Fire and Rescue Department assistant director (operations) Azizan Ismail said the department was alerted of the incident at 7.15pm before a team was deployed to the scene.

"Seven firemen from the Setapak fire station were immediately deployed and checks found that there were no casualties," he said.

Azizan said the team was still in the process of removing the crane late into the night and due to the incident, traffic was backed up.

3 workers injured after 300-ton crane collapses at Exxon Mobil's Torrance refinery

Three workers were injured Monday morning when a 300-ton crane collapsed at Exxon Mobil’s refinery in Torrance, fire officials said.

The contract workers suffered minor injuries during the collapse at the refinery in the 3700 block of West 190th Street, said Battalion Chief Bob Millea of the Torrance Fire Department. The workers were taken to an area clinic for evaluation.

Fire officials said the crane was moving debris at the refinery just after 9:30 a.m. when it suddenly collapsed, he said.

Water was doused onto the debris to stop any vapors from escaping, Millea said. The situation, he said, appeared to be “static.”

The extent of the damage to the refinery was not immediately known, he said.

Exxon Mobil spokesman Todd Spitler said the company was cooperating with fire officials and “determining the cause of the incident.”

A breakdown in some units triggered a flaring event at the refinery, he said. The refinery’s flare system is activated to safety stabilize operations during an unplanned interruption. The flaring event was not related to the collapse, Spitler said.

“Our main concern is for the safety of our employees, contractors and the community,” Exxon Mobil spokeswoman Gesuina Paras said. “There is no impact to the community.”
